Discuss options and follow-up
You and your provider talk through the options that fit your situation, and arrange follow-up care when it is appropriate.
What to have nearby
Nothing needs to be prepared to request an appointment. These simply make the evaluation itself go more smoothly:
- Medication bottles, or an up-to-date list of what you take
- Your pharmacy’s name and phone number
- Your insurance card
- Notes about any questions or concerns you want to raise
- A private, well-lit space and a phone, tablet, or computer
Would you like someone to help?
A trusted family member or caregiver may help with the technology, or take part in the appointment, when the patient requests it and gives permission.
The patient remains the person receiving care. Clinical decisions are made with the patient, and another person cannot authorize care on their behalf without their involvement.
